Here’s what you need to do:

  1. Call us at (701) 671-4478 to confirm coverage by your insurance company
  2. Contact your doctor and ask for a Physical Therapy or Occupational Therapy or Speech Pathology order for “Evaluation and Treatment”
  3. Bring the order with you to your first appointment OR have your doctor fax it to (701) 642-2485.

Outpatient services are covered by most health insurance plans with a physician referral.
